Only trained and authorized personnel are permitted to issue, supervise, and close the permit, maintaining accountability throughout the task. Continuous monitoring during work, along with clear validity periods and closure procedures, ensures conditions remain safe until the job is completed and systems are restored.

Many failures occur not because permits exist, but because they are poorly applied. Skipping risk assessments, relying on generic PPE, ignoring isolation steps, or assuming conditions are safe without testing all undermine the purpose of the permit. Poor communication between workers, supervisors, and HSE teams further increases risk. These weaknesses are often amplified in paper-based systems, where permits can be misplaced, approvals delayed, and audit trails lost.

By enforcing risk assessment, proper controls, correct PPE, isolation, monitoring, and formal sign-off, it plays a vital role in protecting people, assets, and the environment in high-risk industries.
