Most hot-work incidents do not stem from a lack of knowledge; they occur due to lapses in discipline. These weaknesses can be addressed through structured checklists, clear handover rules, defined revalidation triggers, and mandatory post-work inspections that confirm conditions have not deteriorated.

Digital permits standardize required inputs so critical details—such as gas test results, isolations, and monitoring durations—are not overlooked.
